Since April 2020, Living Hope distributes Chronic Medication to the door of over 3000 patients per month

Home Based Care | Achieving the Vision

The Home Based Carers are encouraged to pray with clients during their home visits as they nurse the sick.

Why Do We Serve?

To show practical care to patients in their homes and when appropriate to use the opportunity to share about Jesus.

Who Do We Serve?

Clients of all ages identified by staff, at community outreaches or through household assessments, and clients who have been given a formal reference from a doctor or clinic.

How Do We Serve?

The Home Based Carers go to client’s home to distribute chronic medication, to ensure they are taking their medication, provide bed baths, pray with the clients and identify further opportunities for assistance by doing a household assessment.

Where Do We Serve?

The deep south of Cape Town, including Capricorn, Masiphumelele, Ocean View, Overcome Heights and Red Hill.
