HOME-BASED CARE A Home-Based Care program, led by Living Hope staff members, provides out-patient service with more than 12,000 visits annually. Home-Based Carers treat and dress wounds, monitor patients' health and offer advice on healthy eating and lifestyle changes. They offer support to the families and give medical or social referrals as needed.
